Welcome to the Davis Health Office
New York State requires that a physical exam be on file at the Health Office for all new entrants as well as students in grades K, 1, 3, and 5.

NYS Health Appraisal Form (Physical Form)
Allergy Action Plan (to be completed by a health care provider if your child has any life threatening allergies)
Asthma Form (to be completed by a health care provider)
Seizure Action Plan (to be completed by a healthcare provider if your child has seizures)
Medication Administration Form (School nurses may administer medications to students during the school day with a written prescription from a physician, written permission from the parent/guardian, and the medication in the original or pharmacy-labeled container)
